It's a public secret that Jeffrey Epstein is dead not from suicide, but because he couldn't keep his big mouth shut. Ghislaine knows how to shut up and deny any involvement by powerful people in their little underage girls and blackmail operation, which is why nobody took her out.
The Clintons are commonly accused of being involved--they certainly have a record of involvement in both credible allegations of sexual crimes and inconvenient people around them being “Arkancided”--so people commonly joke about Hillary being the one who had Jeffrey taken out. It's a running joke. There's no reason to particularly think that it was her and not one of the other powerful people who feared being exposed, but she does have a history of being a suspect.
